How to Read a Clinical Trial Record: What the Registry Actually Tells You

A ClinicalTrials.gov record shows you a trial’s current state. It does not show you what changed, when, or how often. A few fields carry most of the signal, and each one means something different from what it appears to say.

What does a trial’s status field actually tell you?

Less than most readers assume. Status is self-reported by the sponsor and updated at the sponsor’s discretion, not on a schedule. It records what the sponsor last said, not what is happening now, and the gap between those two things is where most of the useful signal lives.

That single fact explains the three sections below. A record can read Recruiting for years after recruitment stopped. It can pass its own completion date without anything being updated. It can be quietly abandoned and then flagged automatically. None of that appears as an event, because the registry has no concept of an event. It only has a current state.

What does “unknown status” mean, and should you worry?

It means the sponsor stopped updating the record and its completion date has passed, so ClinicalTrials.gov flagged it automatically. It is a statement about the record, not about the science. The trial may have finished, been abandoned, or still be running with nobody maintaining the entry.

It is far more common than most people expect. As of 3 August 2026, 67,363 studies carry unknown status, which is 14.9% of the interventional registry. Of those, 99.9% have never posted results. Roughly one record in seven is a dead end, and almost none of them will ever tell you how they ended.

Is this trial actually still running?

Often the record cannot tell you. Two checks answer it faster than reading the record itself: whether the trial is past its own posted primary completion date, and when the sponsor last touched the entry.

Among open trials that posted a primary completion date, roughly three in ten are already past it, and the rate barely moves across phases.

A date that has passed with no update is not evidence of failure. It is evidence that nobody has revised the record, which is a different and more useful thing to know.

The second check is staleness. Of the 45,773 trials currently reading Recruiting, 31.2% have not been updated in over twelve months and 7.0% not in over two years. The median recruiting record was last touched 201 days ago. A trial that says it is recruiting today may be reporting a fact from 2024.

Terminated, withdrawn or suspended: what actually follows?

The three are not interchangeable. Withdrawn means the trial never enrolled anyone. Terminated means it started and stopped early. Suspended means it paused and may resume. What they share is silence afterwards.

The last row is the one worth sitting with. Completed trials are the best-reported category in the registry and 76.5% of them still have no results posted. If you are waiting for a competitor’s readout to appear on ClinicalTrials.gov, the base rate says it will not.

Why can’t the ClinicalTrials.gov API tell you this?

Because the API returns a record’s current state, not its history. Two identical responses a week apart cannot tell you whether a field moved and moved back, and a field that changed the day after you polled looks the same as one that never changed at all. AACT, the public research database built on the same source, has the same limitation.

Every figure on this page exists because the changes were recorded as they happened, against a stored history. That is a different kind of dataset from a snapshot, and it is the reason questions like “how often does this actually happen” are hard to answer from the registry alone.

So how do you keep track of what changed?

You need something that keeps the registry’s history, because the registry does not keep it for you. That is either a job you build or one you buy, and it is worth knowing what building it involves: a daily copy of every trial you care about, yesterday’s copy kept alongside it, a comparison between the two, and a rule for which of the resulting differences deserve anyone’s attention. Most of the work is in that last part.
